July 10, 2018 — Initial Investigation

2243 hrs Email received from the SWP requesting I contact Daryl Mosby. I called Mr. Mosby and left a voicemail requesting a return call. 2254 hrs Email notification of report received from SWP. I called Mr. Mosby again and reached his voicemail. I searched E-Plan for an alternate number and there was no answer.

July 11, 2018 — Follow-Up Investigation

0315 hrs NRC Incident Report # 1217951 received via email indicates facility operations have been stopped until a cause can be determined. Per the NRC, one employee was injured and facility personnel have performed a sweep of the area to ensure no product residue remains. 0700 hrs Call received from Mr. Mosby stating an employee was in the chamber area where the sterilization process occurs when the employee observed a small spark/flash. Mr. Mosby stated no release of product was observed and no monitoring equipment indicates a release has occurred. Mr. Mosby stated the cause of the spark is being investigated and he currently has no reason to believe there has been a release. 0730 hrs Text notification of report received from the SWP. I confirmed receipt. This is a non-release report. I recommend closing this report with no further EPD ERT actions required at this time.
